Projects for Assistance in Transition from Homelessness (PATH) is a community outreach program that mirrors HOPE, but serves both civilians and Veterans. It focuses on individuals experiencing or at risk of homelessness, especially those struggling with severe mental illness.

Services Offered:

Outreach Services
PATH teams go directly to where people are, streets, camps, parks, and shelters. Staff observe signs of homelessness and mental health concerns based on appearance, behavior, or speech. These face-to-face interactions help build trust and open the door to services.

Case Management Services
Case Managers meet clients in person to determine eligibility and assess their needs. They develop a personalized plan for each client and collaborate with community partners, when authorized to avoid duplication and ensure coordinated care.

Peer Support Services
Certified Peer Support Specialists offer lived experience and compassion. Many have faced homelessness or mental illness themselves. They provide practical support and mentorship, building authentic peer relationships that reinforce each client’s journey toward stability.
